Why does my outlet say "hot/ground reversed" when it is loaded?

I have a tester in one half of the outlet that says correcet (both orange lights lit). When I plug something into the other plug and turn it on, the appliance doesn't turn on, and the tester switches to hot/ground reversed (two outer lights lit up).
